Young Leaders of Northeast Indiana (YLNI) in partnership with The History Center are pleased to host the Barr Street Market for the 6th year in a row! The market will be open on Saturdays from 9 a.m. to 1 p.m., July 10 through September 25, 2010, and is still located in the heart of downtown Fort Wayne on the corner of Wayne and Barr Streets. Free parking is available in the History Center lot along Berry Street. Bikers are also encouraged to ride and park at a corral within the market.

The Barr St. Market is a producer-only market, meaning that our vendors personally make or grow the products they sell. Every vendor is local, coming from Allen or surrounding counties

The public is invited to attend a health fair Sept. 11 from 9 a.m. to 1:30 p.m. at the Parkview Noble Hospital campus, 401 Sawyer Road, in Kendallville. The event will feature more than 20 booths from departments within the hospital and several community agencies, including CANI. Reduced cost blood tests will also be available. In addition to the health fair, there will be a 5k run/walk and a variety of children’s activities. CANI will provide information on the state-funded health insurance plans available and will assit with the application process. The Healthy Indiana Plan (HIP) is intended for adults ages 19-64 who do not have access to health insurance. Hoosier Healthwise is the free or low cost state-sponsored health plan for low-income families, children up to age 19, and pregnant women. See below for information on the needed documentation for the application process:

Social Security Numbers-for everyone living in the household
Identity-for each applicant and parent or guardian
Valid drivers license, State Identification Card, student ID, Military ID.

If you have someone acting on your behalf, that person will need to provide proof of his or her identity also. If child is under the age of 16 the Identity Affidavit for Children Under Age 16 must be submitted.

US Citizenship-for each applicant
Legal birth certificate, Certificate of Naturalization, Certificate of Citizenship or U.S. passport if it was issued without restrictions.
Money received by applicant, spouse, and dependent children in the home

1. Wages: Pay stubs, paychecks, or statement from employer for the past 30 days. You will need 60 days of pay stubs if you want coverage to go retroactive back 60 days. Hoosier Healthwise can go retroactive back 90 days; therefore you will need 90 days of pay stubs.

2. Self-employment: Last year’s signed tax return or personally kept self-employment records for the past 90 days.

3. Child Support, Social Security, VA, SSI, Workers Compensation, disability, sick pay, unemployment, or other benefits: Court order, award letter, or other proof of payment from the source of income.

4. Loans, gifts or contributions: Promissory note; loan agreement; or statement from the person providing the money that includes the person’s name, address, phone number, signature and date.

5. Financial aid award letter for the current year (if applicant is currently in college and receiving any grant or financial aid).

6. Wage verification from unemployment office (if currently unemployed and not receiving an income).

Guardianship or Power of Attorney If someone has legal authority to act on your behalf, provide a copy of the Power of Attorney, Guardianship Order, Court Order, or similar documents.

Immigration status A copy of your alien registration card, permanent resident card or other documentation from the Bureau for Citizenship and Immigration Services (formerly the INS).

Actor Sean Astin will always be “Mikey” in the popular classic, The Goonies, and will forever be remembered for portraying Samwise Gamgee in The Lord of the Rings trilogy. A steadfast activist and civic-minded individual, Astin has served the administrations of George W. Bush and Bill Clinton and volunteered for many non-profit organizations. He supported his mother, Patty Duke, in her battle with mental illness as a spokesperson on Astra Zeneca’s comprehensive Bi-Polar Awareness Tour.

Astin has appeared on countless local and national radio and television shows including Larry King Live, Good Morning America, The View, and 20/20. A familiar face on the late night circuit, he has appeared on The Late Show with David Letterman, Real Time with Bill Maher, The Tonight Show and The Late Late Show.

In anticipation of a capacity audience for this lecture, tickets will be required. Beginning, Monday, Aug. 30, free tickets must be picked up in person at the Schatzlein Box Office in the lobby of the Rhinehart Music Center, noon-6:30 p.m., Monday through Friday. There is a limit of six tickets per person. Tickets will hold seats until 7:15 p.m. on the day of the event. Thereafter, all unoccupied seats will be released for stand-by patrons.

The IPFW Omnibus Lecture Series brings six distinguished speakers to the university and the northeast Indiana community each year. The series, in its 16th year, is committed to presenting diverse perspectives through educated, respected, and entertaining speakers.

There is no charge for the lectures and no other event this season will require tickets.

Omnibus is made possible with the support of the English, Bonter, Mitchell Foundation and publicized with support from Northeast Indiana Public Radio and WANE-TV NewsChannel 15.

The 2010-2011 Omnibus Lecture Series is held in The John and Ruth Rhinehart Music Center in the acoustically superb, 1,600-seat Auer Performance Hall. Doors open at 6:30 p.m. The music center is located on the north end of campus in the arts plaza next to Williams Theatre and the Visual Arts Building, and behind the Medical Education Center.

Due to campus construction, attendees should plan to arrive early, allowing additional time for traffic and parking. Only drop-off and eight handicapped parking spaces will be available directly in front of the music center building.

A Goshen cancer researcher will give the keynote address at the 2010 Health Care Hero Awards breakfast at 7:30 am, Friday September 17th. The Event will be held at Don Hall's Guesthouse, 1313 W. Washington Center Rd. in Fort Wayne.

Douglas J. Schwartzentruber, M.D., is a surgical oncologist and medical director of the Center for Cancer Care in Goshen, and earlier this year was named one of Time magazine’s 100 most influential people in the world for his work in cancer research. He is a native of Goshen, a graduate of Goshen College and the Indiana University School of Medicine and has completed fellowships in immunotherapy and surgical oncology. He was a senior investigator at the National Cancer Institute for 13 years before returning to Goshen.

The Health Care Hero Awards honor organizations and individuals in the health care field who epitomize the spirit embodied in the word "hero."

A winner and a runner-up will be honored in each of the following categories: Community Achievement in Health Care, Physician, Advancement in Health Care, Medical Professional and Volunteers.

Award winners will be honored at the Health Care Hero Awards Breakfast, profiled in a special publication published by the Greater Fort Wayne Business Weekly and will receive the distinctive Health Care Hero Award.

Fort Wayne, Ind.) – United Way of Allen County and Organized Labor have scheduled their annual Senior Health Fair for Friday, October 1, 2010, from 8 a.m. until noon at Turnstone Center for Children and Adults with Disabilities, 3320 N. Clinton St. The 18th Annual Senior Health Fair is sponsored by Parkview Home Health & Hospice and everyone, regardless of age, is invited to attend.

Free blood pressure, glucose and colorectal cancer screenings will be available. A blood chemistry test is also available for a $17 fee; this tests liver function, kidney function, cholesterol levels, high density lipoproteins (HDLs), low density lipoproteins (LDLs), triglycerides and more.

Also during the event, Parkview Home Health & Hospice will give flu shots for $21 and Pneumonia vaccines for $35. Anyone covered by Medicare Part B will not need to pay for these shots.

For the second year, Francine's Mobile Mammography will be on site from 8 a.m. to 3 p.m. to provide mammograms. To schedule an appointment, women should call 260-483-1847. Some walk-ins will be taken, depending on the schedule that day.

Many health care organizations, such as Aging & In-Home Services, the Arthritis Foundation, Alzheimer’s Foundation and more, will be on hand with information and will have health care professionals available to answer questions.
